Travel Social Worker (Hospital & ED) – 13-week travel contract in Fremont, CA. Immediate openings for hospital social workers with MSW/LCSW and EPIC experience who want to join a fast-paced emergency department team in the Bay Area.

• Location: Fremont, CA (94538) — East Bay / Bay Area / near Silicon Valley
• Role: Travel Social Worker | Hospital Social Worker | Emergency Department (ED) Social Worker
• Employment Type: Travel Contract (On-Site)
• Contract Duration: 13 weeks (with potential for extension)
• Estimated Weekly Pay: $2,788 - $3,097 (competitive travel social work pay)
• Hours per Week: 40
• Shifts per Week: 4
• Shift Pattern: 10-hour day shifts (rotating); PM ED role: 5:00pm – 3:30am; flexibility required

Qualifications & Licensure

• Minimum 1 year of hospital social work experience (ED or inpatient experience preferred)
• MSW or LCSW preferred; CA licensure or ability to obtain California licensure preferred
• Experience with EPIC or similar electronic medical record (EMR) systems required
• Strong assessment, crisis intervention, case management, discharge planning, and community resource referral skills
• Comfort working in a high-volume emergency department and collaborating with interdisciplinary teams
• Ability to work rotating days, evenings, and weekends as needed

Key Responsibilities

• Provide comprehensive hospital social work and case management services with a primary focus in the Emergency Department
• Perform psychosocial assessments and develop individualized care plans in coordination with physicians, nurses, and case managers
• Deliver crisis intervention, patient advocacy, and support for complex medical and behavioral health issues
• Coordinate discharge planning, arrange post-discharge resources, and connect patients to community-based referrals to ensure safe transitions of care
• Document interventions, assessments, and communications accurately and promptly in EPIC/EMR
• Address social determinants of health, identify barriers to care, and collaborate with community partners for ongoing support
