Job Requirements

• Master of Social Work (MSW) or higher required
• At least 1 year of social work experience required
• Liver transplant experience required
• Strong case management, medical social work, and patient advocacy skills
• Excellent communication and interdisciplinary collaboration abilities
• Ability to support patients and families through complex medical and psychosocial needs

Responsibilities

• Provide psychosocial assessments and ongoing support for patients and families
• Assist with care coordination and discharge planning in collaboration with the healthcare team
• Support patients navigating liver transplant-related care and treatment processes
• Connect patients and families with appropriate community resources and support services
• Advocate for patient needs while maintaining accurate and timely documentation
• Collaborate with interdisciplinary teams to promote quality outcomes and continuity of care
