Physical Therapist Job Requirements

• Active Physical Therapist license in the applicable state
• At least 1 year of professional physical therapy experience
• Home health physical therapy experience strongly preferred
• Required proficiency with OASIS assessments
• Required experience with HCHB home health software
• Ability to travel throughout the assigned home health service area
• Strong clinical judgment, patient communication, organization, and clinical documentation skills
• Ability to meet a productivity expectation of 30 points

Home Health Physical Therapist Responsibilities

• Evaluate home health patients based on physician orders and referrals using appropriate assessment tools and clinical procedures.
• Review medical records, diagnoses, precautions, contraindications, and relevant medical history before initiating physical therapy care.
• Develop individualized physical therapy treatment plans with measurable goals focused on mobility, function, safety, and independence.
• Provide skilled physical therapy interventions in the home and regularly reassess patient progress and treatment effectiveness.
• Modify goals and treatment plans as clinically appropriate and coordinate discharge planning.
• Communicate patient needs, progress, and recommendations to physicians, medical providers, family members, and the interdisciplinary care team.
• Recommend and incorporate adaptive equipment, therapeutic exercises, and functional strategies to support patient independence.
• Complete timely and accurate evaluation, treatment, OASIS, and billing documentation using HCHB and required formats.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with applicable practice standards.
• Participate in team meetings, staff development, continuing education, and quality-focused initiatives.
• Maintain compliance with professional standards, privacy requirements, and applicable federal and state regulations.
