Home Health Physical Therapist Requirements

• At least 1 year of physical therapy experience required; 2 years preferred
• Adult health care experience required
• Home health physical therapy experience preferred
• Active Maryland physical therapist license or compact license in hand at the time of submission
• Current Basic Life Support certification
• Experience supporting patients with orthopedic, neurological, geriatric, oncology, and cardiopulmonary needs is valued
• Ability to work independently, manage a daily home health visit schedule, and provide patient-centered care in home environments
• Two professional supervisor references required

Home Health Physical Therapy Responsibilities

• Evaluate adult and geriatric patients in the home setting and develop individualized physical therapy treatment plans
• Provide skilled physical therapy interventions to improve strength, balance, mobility, transfers, and functional independence
• Complete approximately 6–7 patient visits per day while maintaining accurate clinical documentation
• Educate patients and caregivers on therapeutic exercises, home safety, mobility techniques, and strategies for continued progress
• Monitor patient responses to treatment and modify care plans as clinically appropriate
• Collaborate with interdisciplinary support services, including social services and interpretation services, to promote comprehensive patient care
• Communicate effectively with patients, caregivers, and members of the care team regarding progress and ongoing needs
