Job Description
Join a 13-week travel contract as a Case Manager supporting utilization review and discharge planning across inpatient, observation, and swing bed settings. This non-clinical role includes reviewing NOAs, managing a caseload of approximately 8–10 cases per day, coordinating discharge planning, and working with insurance portals. You will use Meditech and collaborate with pharmacy resources for discharge planning. Experience with COPD, CHF, or debility-related swing bed cases is helpful. Candidates must have at least two full years of experience and BLS certification; CCM certification is preferred but not required. A market Care Manager will serve as a mentor throughout the assignment. Enjoy a consistent weekday schedule with no weekends, holidays, or call.
