Job Description
Join this 13-week travel healthcare contract as a Case Manager Registered Nurse in Oakland, California. You will develop and document safe discharge plans, support transitions of care, coordinate post-acute placements and services, and help guide patients through the next steps of their care journey. The assignment includes floating across inpatient units and working every other weekend, offering a varied case-management experience. This role requires at least three years of acute-care experience, familiarity with EPIC, and experience with ambulatory, home health, hospice, SNF, ANU, and LTACH referrals. A California RN license, ADN, and BLS certification are required.
