Occupational Therapist (OT) – Home Health Contract in Clifton, NJ

Home Health Occupational Therapist Job Details

• Location: Clifton, New Jersey, with home health service coverage throughout Passaic and Bergen counties
• Weekly Estimated Pay: $1,610–$1,730
• Start Date: November 2, 2026
• Contract End Date: February 21, 2027
• Duration: Approximately 16 weeks
• Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m.
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week
• On-Call: No on-call requirement
• Productivity Expectation: 30 points per week
• Mileage: Mileage reimbursement available at $0.50 per mile

Home Health Occupational Therapist Requirements

• Current Occupational Therapist license in the state of New Jersey; compact licensure is accepted
• At least 1 year of occupational therapy experience, including home health experience
• Experience developing and implementing individualized occupational therapy treatment plans
• Working knowledge of home health documentation, patient assessments, and care coordination
• HCHB experience required
• OASIS and PointCare experience preferred
• Strong communication, organization, clinical judgment, and time-management skills
• Ability to travel between patient visits throughout the assigned Passaic and Bergen county service area

Home Health Occupational Therapist Responsibilities

• Evaluate patients in their homes or alternative care settings to identify functional needs and occupational therapy goals
• Develop individualized treatment plans in collaboration with physicians and the interdisciplinary care team
• Provide therapeutic exercises and interventions designed to improve mobility, coordination, strength, and independence
• Educate patients and caregivers on activities of daily living, adaptive techniques, home safety, and continued progress
• Document evaluations, treatment sessions, progress, and care plans accurately and promptly in accordance with home health standards
• Monitor patient response to treatment and modify interventions as clinically appropriate
• Coordinate care with other healthcare professionals to support continuity of care and positive patient outcomes
• Supervise assigned Occupational Therapy Assistants in accordance with applicable standards and regulations
