Home Health LPN (Licensed Practical Nurse) – 13-Week Contract | Peoria, AZ

Home Health LPN Job Details

• Location: Peoria, Arizona, with home health visits throughout Phoenix, Glendale, Goodyear, Litchfield, Litchfield Park, Surprise, Tolleson, Buckeye, Youngtown, Sun City, Sun City West, and Anthem
• Healthcare Setting: Home Health
• Weekly Estimated Pay: $1,370–$1,440
• Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.; 40 hours per week
• Shift Duration: 8-hour shifts
• Contract: 13 weeks
• Start Date: October 6, 2026
• End Date: January 5, 2027
• Additional Expectations: Weekend and on-call availability required; mileage between patient visits is reimbursed
• Dress Code: Any scrub color

Home Health LPN Qualifications

• Current Arizona Licensed Practical Nurse (LPN) license
• At least 1 year of professional LPN experience required; home health nursing experience strongly preferred
• Arizona Level I Fingerprint Clearance Card
• Current CPR certification
• Associate degree or equivalent education
• Strong patient-care focus, communication skills, and the ability to work independently in the field
• Willingness to travel throughout the assigned home health service area

Home Health LPN Responsibilities

• Provide skilled follow-up and subsequent nursing visits for home health patients
• Deliver compassionate, individualized care while promoting patient safety, recovery, and independence
• Perform wound care, wound vacuum care, compression wraps, and catheter changes
• Provide tracheostomy care, nephrostomy tube care, nasogastric tube care, and urinalysis collection as ordered
• Administer medications as ordered, reinforce medication-related education, and assist patients with medication management
• Educate patients and families regarding disease processes, treatment plans, symptom monitoring, and self-care
• Document patient assessments, interventions, progress, and communication in accordance with home health standards
• Meet productivity expectations of approximately 30 points while maintaining high-quality patient care
• Coordinate care with the interdisciplinary healthcare team and report changes in patient condition promptly
