Join a dedicated home health team as a Physical Therapist serving patients throughout the San Antonio area. In this role, you will provide individualized physical therapy in patients’ homes and alternative care settings, helping individuals improve mobility, independence, and activities of daily living. San Antonio’s 78232 area offers access to a broad and diverse community, while the regional service area provides opportunities to make a meaningful impact across multiple surrounding counties.

Physical Therapist Job Details

• Position: Home Health Physical Therapist (PT)
• Setting: Home health and alternative care settings
• Location: San Antonio, Texas 78232, with coverage across Bexar, Medina, Wilson, Comal, Kerr, Atascosa, Bandera, Kendall, and Guadalupe counties
• Contract: 13 weeks
• Start Date: November 2, 2026
• End Date: January 31, 2027
• Schedule: 8-hour shifts, Monday through Friday, 8:00 a.m. to 5:00 p.m.
• Hours: 40 guaranteed hours per week
• On-Call: One weekend per month
• Weekly Estimated Pay: $1,560–$1,670
• Productivity Expectation: 30 points
• Required Documentation System: HCHB

Home Health Physical Therapist Requirements

• Current Physical Therapist license in Texas
• Experience providing care in home health, home-based care, or a comparable community-based setting preferred
• Proficiency with HCHB documentation required
• Ability to travel throughout the assigned service area and provide reliable patient visits
• Strong communication, organization, clinical judgment, and time-management skills

Home Health Physical Therapist Responsibilities

• Evaluate patients in their homes or alternative care settings and identify functional limitations and skilled therapy needs
• Develop, implement, and update individualized physical therapy treatment plans based on patient goals and physician-approved plans of care
• Provide therapeutic exercises and skilled interventions to improve strength, balance, mobility, and activities of daily living
• Instruct and motivate patients and caregivers on safe mobility techniques, exercise programs, and home-management strategies
• Assess and recommend appropriate adaptive equipment and support its safe and effective use
• Provide clinical supervision and oversight of Physical Therapist Assistants and Home Health Aides as appropriate
• Document assessments, interventions, patient progress, and care-plan updates accurately in HCHB
• Collaborate with physicians and the interdisciplinary care team to promote coordinated, high-quality patient care
