MSW / Case Manager Job Details

• Location: Mooresville, North Carolina
• Weekly Estimated Pay: $1,821–$2,023
• Assignment Duration: 13 weeks
• Targeted Start Date: November 16 or November 30, 2026
• Schedule: Sunday–Thursday, 8:00 a.m.–4:30 p.m.
• Shifts: 5 shifts per week
• Shift Duration: 8-hour days
• Healthcare Setting: On-site healthcare environment
• Charting System: EPIC
• On-Call: Not required
• Orientation: Biweekly

Medical Social Worker Job Requirements

• Master of Social Work (MSW) degree required
• At least 2 years of professional social work or case management experience
• Strong discharge planning experience
• Experience working in a healthcare setting preferred
• Ability to document accurately and efficiently in EPIC
• Flu vaccination is required; exemptions and declinations are not accepted
• Excellent communication, organization, assessment, and care coordination skills

Social Work and Case Management Responsibilities

• Provide comprehensive medical social work and case management services to patients and families
• Assess psychosocial needs, barriers to care, and discharge readiness
• Develop and coordinate individualized discharge plans
• Collaborate with interdisciplinary care teams, patients, families, and community resources
• Coordinate referrals, post-acute services, and transitions of care
• Document assessments, interventions, care plans, and outcomes in EPIC
• Advocate for patient needs while supporting safe, timely, and appropriate transitions from care
