• Shift: 8-hour evening shifts, 12:00 p.m. to 8:30 p.m.
• Schedule: Per diem; no guaranteed weekly hours
• Weekend/Holiday Coverage: As needed
• On-Call Requirements: None
• Float Requirement: Ability to work between nearby assigned locations approximately 25 miles apart
• Orientation: 8 hours

LPN Job Requirements

• Current Minnesota Licensed Practical Nurse (LPN) license
• At least 1 year of long-term care experience; 2 years of nursing experience preferred
• Current AHA Basic Life Support (BLS) certification
• Associate degree or practical nursing certificate
• COVID-19 vaccination required; applicable exemptions will be considered
• Strong organizational skills and the ability to complete tasks independently and on time
• Ability to float between assigned care locations as needed
• Matrix charting and electronic health record experience preferred
• Commitment to wearing work-appropriate solid colors or prints

Long-Term Care LPN Responsibilities

• Provide compassionate, safe, and individualized nursing care to long-term care residents
• Administer medications and treatments according to established orders and procedures
• Complete resident assessments, nursing documentation, and electronic charting accurately and promptly
• Monitor resident conditions and communicate changes to the appropriate care team members
• Support activities of daily living while promoting resident dignity, comfort, and independence
• Respond appropriately to resident needs, urgent concerns, and changes in condition
• Collaborate with nursing assistants, TMAs, providers, residents, and families
• Manage assigned responsibilities efficiently in a busy long-term care environment
• Float between nearby care locations and adapt to changing staffing and resident-care needs
