Physical Therapist Assistant (PTA) – Home Health | Frankfort, Kentucky

Home Health PTA Job Details

• Weekly Estimated Pay: $1,470–$1,580
• Location: Frankfort, Kentucky, with patient visits throughout Woodford, Mercer, and Anderson counties
• Healthcare Setting: Home health
• Schedule: Monday through Friday, approximately 8:00 a.m. to 4:30 p.m.
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week
• Start Date: October 5, 2026
• Assignment Duration: Approximately 16 weeks, through January 25, 2027
• Weekend/On-Call: No regular weekend or on-call requirement indicated
• Additional Support: Mileage reimbursement between patient visits is provided
• Dress Code: Solid-color scrubs

Physical Therapist Assistant Job Requirements

• Associate degree from an accredited Physical Therapist Assistant or physical therapy assistant program
• Current Physical Therapist Assistant licensure or authorization to practice in Kentucky
• At least 1 year of experience in home health, outpatient rehabilitation, or a comparable physical therapy setting
• Strong clinical judgment, organization, and time-management skills
• Ability to travel independently between patient homes throughout the assigned service area
• Excellent patient communication and clinical documentation skills
• Experience with home health care documentation systems is preferred

Home Health Physical Therapy Responsibilities

• Provide physical therapy treatment under the direction and supervision of a licensed Physical Therapist
• Deliver individualized home health rehabilitation services in patients’ homes
• Assist patients with therapeutic exercises, mobility training, transfers, balance activities, and functional tasks
• Monitor patient progress and communicate changes in condition to the supervising Physical Therapist and interdisciplinary care team
• Educate patients and caregivers on safe mobility techniques, home exercise programs, and strategies that support independence
• Complete timely and accurate clinical documentation in accordance with home health standards
• Coordinate patient visits efficiently across Woodford, Mercer, and Anderson counties
• Support weekly productivity expectations of approximately 30 points while maintaining high-quality patient care
