Registered Nurse Job Requirements

• Active New Jersey Registered Nurse (RN) license
• At least 2 years of recent home health nursing or home care experience
• Strong Medical-Surgical nursing skills
• OASIS assessment and documentation experience
• Experience using EPIC electronic medical records (EMR)
• Valid driver’s license
• Reliable transportation for in-home patient visits
• Strong clinical judgment, communication, organization, and documentation skills

Home Health Registered Nurse Responsibilities

• Provide skilled nursing care to patients in their homes throughout the Mount Holly/Marlton area
• Perform comprehensive nursing assessments and monitor patient condition and progress
• Develop, implement, and update individualized plans of care
• Apply Medical-Surgical nursing knowledge to manage a variety of patient needs
• Complete accurate and timely OASIS assessments and clinical documentation in EPIC
• Administer medications and treatments in accordance with the plan of care
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
• Collaborate with providers and interdisciplinary care team members to coordinate home health services
• Recognize changes in patient condition and communicate concerns promptly
• Provide dependable coverage for assigned weekend and holiday rotations
