Physical Therapist (PT) – Home Health | Newton, Massachusetts

Home Health Physical Therapist Job Details

• Setting: Certified home health
• Location: Newton, Massachusetts, with travel to assigned patient cases
• Start Date: November 22, 2026, pending credentialing clearance
• Contract Duration: 13 weeks
• Schedule: 40 hours per week; 8-hour day shifts, generally 8:00 a.m. to 4:30 p.m.
• Weekend Rotation: Approximately once every 6 weeks
• Weekly Estimated Pay: $2,260–$2,430

Physical Therapist Job Requirements

• Current Massachusetts Physical Therapist license or eligibility to practice
• At least 1 year of experience providing physical therapy in a certified home health or certified home care setting
• At least 1 year of case management experience in a certified home health agency
• Home health experience must be gained within a certified home health agency; hospice, private care, or case management experience alone does not meet this requirement
• Ability to travel independently to assigned patient cases throughout Newton and the surrounding Boston area
• Strong clinical judgment, patient communication, organization, and electronic documentation skills
• Ability to complete required credentialing and onboarding requirements before beginning the assignment

Home Health Physical Therapist Responsibilities

• Evaluate patients in their home environment and assess functional mobility, strength, balance, and safety needs
• Develop, implement, and update individualized physical therapy plans of care
• Provide skilled physical therapy interventions to improve mobility, independence, and quality of life
• Educate patients and families on therapeutic exercises, mobility techniques, fall prevention, and the safe use of assistive devices
• Monitor patient progress and modify treatment plans as clinically appropriate
• Perform admission and ongoing case management activities in accordance with home health standards of practice
• Maintain thorough, timely electronic documentation and communicate effectively with the interdisciplinary care team
• Coordinate care and support continuity of services across the patient’s home health episode
