Home Health Physical Therapist (PT) Contract – Brunswick, Georgia

Home Health Physical Therapist Job Details

• Location: Brunswick, Georgia, and surrounding communities
• Weekly Estimated Pay: $2,100–$2,250
• Schedule: Monday through Friday
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week with guaranteed hours
• Start Date: November 23, 2026
• Contract Duration: 13 weeks, ending February 21, 2027
• On-Call: None
• Service Area: Glynn, Charlton, Brantley, and Camden counties
• Productivity Expectation: 30 points
• Mileage: Mileage reimbursement provided at $0.49 per mile

Physical Therapist Job Requirements

• Current Physical Therapist license; compact licenses accepted
• At least 1 year of professional physical therapy experience
• Previous home health physical therapy experience required
• Proficiency with HCHB documentation and workflow preferred
• Strong clinical judgment, communication, organization, and time-management skills
• Ability to travel throughout the assigned service area and provide skilled care in patients’ homes

Home Health Physical Therapy Responsibilities

• Evaluate patients in their homes or alternative care settings and identify functional needs
• Develop, implement, and update individualized treatment programs based on physician-approved plans of care
• Provide therapeutic exercises and skilled physical therapy interventions to improve strength, balance, mobility, and activities of daily living
• Instruct and motivate patients and caregivers in safe mobility techniques, exercise programs, and home care strategies
• Assess and recommend appropriate adaptive equipment and support its safe, effective use
• Provide clinical supervision and oversight to Physical Therapist Assistants and Home Health Aides as assigned
• Document evaluations, treatments, progress, and outcomes accurately and promptly in HCHB
• Communicate effectively with physicians and the interdisciplinary care team to coordinate patient-centered home health care
• Monitor patient progress and modify treatment plans as clinically appropriate
