• Hours per Week: 40
• Shift Duration: 8-hour shifts
• Shift Schedule: Day shift, approximately 8:00 AM to 4:30 PM
• Weekly Estimated Pay: $2,030–$2,190
• Coverage Area: Greenbrier, Pocahontas, and Monroe counties
• Weekend/On-Call Requirement: Rotating weekends shared among PTs; no on-call hours
• Productivity Expectation: 30 points
• Dress Code: Navy blue

Physical Therapist Job Requirements

• Current Physical Therapist license required
• Bachelor’s degree required
• Minimum of 1 year of PT experience preferred
• Experience in home health, outpatient rehab, or skilled therapy settings is highly valued
• Strong communication, organization, and time-management skills
• Ability to work independently and manage a patient schedule across multiple locations
• Professional certification is required for this assignment

Physical Therapy Responsibilities

• Provide high-quality physical therapy care to patients in the home health setting
• Evaluate patient needs and develop individualized treatment plans
• Deliver skilled therapy interventions to improve mobility, strength, balance, and functional independence
• Document patient progress accurately and timely in accordance with care standards
• Collaborate with the interdisciplinary care team to support coordinated patient outcomes
• Manage a daily schedule efficiently across the assigned coverage area
• Participate in the rotating weekend coverage schedule as assigned
• Maintain a professional and patient-focused approach in every visit
