• Schedule: Monday through Friday, 8:00 AM - 4:30 PM
• Shift Duration: 8 hours
• Guaranteed Hours: 40 hours per week
• Setting: Home health and hospice
• Travel Area: Hudson County, NJ

Job Requirements

• Current New Jersey Certified Home Health Aide (CHHA) certification required
• CPR certification required
• At least 1 year of experience in home health, private duty, or related patient care preferred
• Ability to support patients with activities of daily living (ADLs) in a home or care setting
• Comfort with HCHB, Microsoft Teams, and Outlook preferred
• Reliable, compassionate, and able to work independently while following care plans

Responsibilities

• Assist elderly patients, convalescents, and individuals with disabilities with activities of daily living
• Provide personal care support in the patient’s home or in a care facility as needed
• Help with light housekeeping tasks such as making beds, laundry, and dishwashing
• Prepare simple meals and support routine nutrition and cleanliness needs
• Offer respectful guidance to families and patients regarding household routines and daily support
• Document and communicate care updates as directed by the care team
• Attend weekly case conference meetings and maintain reliable attendance
