Job Details

• Primary Unit Coverage: Med/Surg

• Duration: 13 weeks
• Schedule: Full-time, 5 shifts per week
• Shift Duration: 8 Hour Days
• Weekend Requirement: Every 3rd or 4th weekend
• Weekly Estimated Pay: $1,575 - $1,750
• Work Arrangement: On-site

Job Requirements

• Minimum 2 years of social work experience required
• Acute care, hospital social work, or medical social work experience required
• Active New Jersey LSW license required
• Experience supporting Med/Surg patients strongly preferred
• Strong communication, care coordination, and interdisciplinary collaboration skills
• Ability to manage psychosocial assessments and discharge planning in a fast-paced setting
• Comfort with weekend rotation requirements

Responsibilities

• Provide social work support to Med/Surg patients and their families
• Conduct psychosocial assessments and identify barriers to discharge
• Assist with discharge planning and coordination of post-acute care services
• Collaborate with nursing, case management, physicians, and interdisciplinary care teams
• Connect patients and families with community resources, placement options, and support services
• Support family communication and patient advocacy throughout the care continuum
• Maintain timely, accurate documentation within the hospital EMR
• Participate in weekend rotation as required
