• Assignment Length: 13 weeks
• Shifts Per Week: 3 (12-hour days, 5:00am – 5:30pm)
• Guaranteed Hours: 36 hours per week
• Weekend & Holiday Requirements: Every other weekend and holiday
• Orientation: 12 hours
• Scrub Color: Any professional color

Job Requirements / Qualifications

• Active LPN license in the state of Illinois (required)
• Minimum 1 year of LPN experience; geriatric/long-term care experience preferred
• BLS Certification (required)
• Valid Driver’s License
• Experience with PointClickCare (PCC) EMR preferred — experience with EMR documentation/clinical charting is a plus
• Comfortable with medication administration, wound care, and IV antibiotics/fluids (no titratable drips)
• First-time travelers and local LPN candidates are welcome

Responsibilities / Clinical Duties

• Provide compassionate, resident-centered nursing care in a long-term care and skilled nursing environment
• Perform assessments, administer medications, and document care accurately in the PointClickCare (PCC) EMR system
• Manage wound care including dressing changes and wound vacs
• Administer and monitor IV antibiotics and fluids (no titratable drips)
• Collaborate with interdisciplinary team members including PT/OT/SLP, respiratory therapists, social workers, and dietitians to support holistic resident care
• Float between units as needed with efforts to maintain consistency during stretches
• Participate in grand rounds, team meetings, and care planning sessions to improve outcomes
