Home Health Physical Therapist Job Details

• Weekly Estimated Pay: $2,260–$2,430
• Start Date: November 2, 2026
• Contract End Date: January 31, 2027
• Contract Duration: 13 weeks
• Schedule: 8-hour day shifts, Monday–Friday
• Hours: 40 hours per week
• Service Area: Everett, Lynnwood, Edmonds, Mountlake Terrace, Bothell, southern Snohomish County, and Mukilteo
• Work Environment: On-site, field-based home health visits within approximately a 50-mile radius of the service area

Physical Therapist Job Requirements

• Current Physical Therapist license eligible for practice in Washington State
• At least 1 year of home health Physical Therapist experience
• Experience providing skilled physical therapy services in patients’ homes and coordinating care across a community-based caseload
• HCHB experience is required
• Availability to travel throughout the assigned service area, including locations within a 50-mile radius
• Strong clinical judgment, communication, organization, and clinical documentation skills

Home Health Physical Therapy Responsibilities

• Evaluate patients in their homes and develop individualized physical therapy plans of care
• Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, and functional independence
• Educate patients and caregivers on home exercise programs, safety strategies, mobility techniques, and disease-specific needs
• Collaborate with nurses, physicians, occupational therapists, speech-language pathologists, and other members of the interdisciplinary care team
• Manage a community-based caseload with a minimum productivity expectation of 30 points per week
• Complete accurate documentation in HCHB by 10:00 a.m. the following day
• Monitor patient progress, update treatment plans, and communicate changes in condition or care needs
• Promote safe transitions of care and support patients in reaching their highest achievable level of function
