Physical Therapist Job Details

• Location: Paducah, Kentucky; service territory within approximately 50 miles of the local office
• Healthcare Setting: Home health
• Weekly Estimated Pay: $1,760–$1,890
• Schedule: Monday–Friday, approximately 8:00 a.m.–5:00 p.m.
• Shift Duration: 8-hour days
• Hours: Approximately 40 hours per week
• Assignment Length: 13 weeks
• Expected Start Date: October 26, 2026
• Productivity Expectation: 30 points per week

Physical Therapist Job Requirements

• Valid Physical Therapist license in Kentucky
• At least 1 year of professional Physical Therapy experience
• Previous home health Physical Therapy experience required
• Previous experience with HCHB documentation required
• Ability to travel throughout the assigned service territory
• Strong clinical judgment, communication, organization, and time-management skills

Home Health Physical Therapist Responsibilities

• Evaluate patients in their homes and develop individualized plans of care
• Provide skilled Physical Therapy interventions to improve strength, mobility, balance, coordination, and functional independence
• Perform therapeutic exercises, gait training, transfer training, and other clinically appropriate interventions
• Educate patients and caregivers on home exercise programs, safety techniques, and strategies to support progress
• Monitor patient outcomes and modify treatment plans based on clinical response
• Document evaluations, visits, progress, and care coordination accurately in HCHB
• Collaborate with the interdisciplinary care team to support safe, effective, and patient-centered home health care
