Home Health Physical Therapist Job Details

• Location: New Braunfels, TX
• Healthcare Setting: Home Health
• Position: Travel Physical Therapist (PT)
• Weekly Estimated Pay: $2,030–$2,190
• Contract: 13 weeks
• Start Date: October 12, 2026
• End Date: January 10, 2027
• Schedule: Approximately 40 hours per week; 8-hour shifts, typically 8:00 a.m.–4:30 p.m.
• Weekly Hour Guarantee: 36 hours
• Coverage Area: Approximately a 60-mile radius around New Braunfels
• Call/Weekend Requirement: No regular on-call or weekend requirement
• Travel: Mileage between patient visits is reimbursed

Physical Therapist Job Requirements

• Bachelor’s degree in Physical Therapy or a related field
• Valid authorization to practice as a Physical Therapist in Texas
• Professional certification or licensure required for practice
• At least 1 year of experience as a Physical Therapist
• Strong patient assessment, clinical documentation, communication, and care-planning skills
• Home health Physical Therapy experience preferred
• Experience with Homecare Homebase (HCHB) is a plus
• Ability to travel throughout the assigned service area and provide skilled care in patients’ homes

Home Health Physical Therapist Responsibilities

• Provide skilled Physical Therapy services to patients in their homes
• Evaluate patients’ mobility, strength, balance, functional abilities, and rehabilitation needs
• Develop and implement individualized Physical Therapy treatment plans aligned with patient goals
• Perform therapeutic exercises, functional training, mobility interventions, and patient education
• Monitor patient progress and modify treatment plans as clinically appropriate
• Educate patients and caregivers on home exercise programs, safety techniques, and strategies that support independence
• Document assessments, interventions, progress, and care coordination activities accurately and promptly
• Collaborate with patients, caregivers, nurses, physicians, and other members of the healthcare team
