Physical Therapist (PT) – Home Health Contract in Red Bud, Illinois

Physical Therapist Contract Job Details

• Location: Red Bud, Illinois, with travel throughout nearby coverage areas
• Weekly Estimated Pay: $1,740–$1,870
• Contract Dates: November 2, 2026 through February 28, 2027
• Duration: Approximately 17 weeks
• Schedule: Monday through Friday, 8-hour shifts, typically 8:00 a.m. to 4:30 p.m.
• Hours: Approximately 40 hours per week, with a 36-hour weekly guarantee
• Healthcare Setting: Home health
• Travel: Local travel between patient visits; mileage between visits is reimbursed
• On-Call/Weekend Requirement: None required

Physical Therapist Job Requirements

• Active Physical Therapist license in the state of Illinois
• Master’s degree in Physical Therapy or a related field
• At least 1 year of professional physical therapy experience, including home health experience
• Strong clinical assessment, documentation, and patient education skills
• Current certification and credentials required to practice as a Physical Therapist in Illinois
• Valid driver’s license and reliable transportation for community-based patient visits
• Home health documentation experience, HCHB experience, and OASIS competency or certification are highly valued

Home Health Physical Therapist Responsibilities

• Evaluate patients in their homes and develop individualized physical therapy treatment plans based on clinical needs and functional goals
• Provide skilled physical therapy interventions to improve strength, balance, mobility, endurance, and independence
• Educate patients and caregivers on therapeutic exercises, mobility techniques, home safety, and the use of assistive devices
• Monitor patient progress, modify treatment plans as appropriate, and communicate changes with the interdisciplinary care team
• Complete accurate, timely clinical documentation and required home health assessments
• Coordinate care across assigned communities, including Red Bud, Belleville, Waterloo, Chester, O’Fallon, and surrounding Southern Illinois areas
• Supervise and collaborate with Physical Therapist Assistants in accordance with professional and state requirements
• Maintain a compassionate, patient-centered approach during every home health visit
