RN Care Manager – In Home Acute Care / Hospital at Home Program
You will be required to travel to patients’ homes throughout the boroughs, but a company credit card is provided to you to use for Uber.

Our client is seeking an RN Care Manager to join their team in New York, NY.

Schedule (2 options):
- Full time, 4 10’s per week, 7am-5pm, rotating every 3rd weekend
- Full time, 4 10’s per week, 12pm-10pm, rotating every 3rd weekend

- Interact with an interdisciplinary team to help coordinate in-home services
- Help patients recover from acute illnesses at home
- Administer prescribed medications (typically intravenously), obtains blood samples, insert foley catheters and provides therapeutic interventions
- Educate patients, caregivers and family members on in-home best practices
- Help develop individualized care plans and ensure appropriate follow up through the care continuum
- Associate’s Degree in Nursing (RN), Bachelor’s Degree in Nursing (BSN) preferred
- An active Registered Nurse license in the state of New York (RN)
- Current CPR certification
- 1+ year of acute care/hospital based experience (Med/Surg, ED, ICU, etc.)
- 1+ year of in-home case management / care coordination experience, preferred
