This position is based in New York, NY or you can float between New York, NY, Morningside, NY and Queens, NY, dependent on preference.

Our client is seeking an RN Care Coordinator to join their team in New York, NY.

Schedule:

• Full time, 4 10’s, 12pm-10pm, rotating every 3rd weekend

Compensation: $50-$58/hr, dependent on experience, plus a 10% performance bonus

RN Care Coordinator Responsibilities:

• Interact with an interdisciplinary team to help coordinate in-home services and transitional care services
• Communicate discharge information to clinical and administrative staff inside and outside of the Hospital
• Provide telephonic care management services
• Help patients recover from acute illnesses  
• Educate patients, caregivers and family members on in-home best practices
• Help develop individualized care plans and ensure appropriate follow up through the care continuum

RN Care Coordinator Qualifications:

• Associate’s Degree in Nursing (RN)
• Bachelor’s Degree in Nursing (BSN), preferred
• An active Registered Nurse license in the state of New York (RN)
• 1+ year of acute care experience (Med/Surg, ICU, Emergency Medicine, etc.)
• 1+ years of case management / care coordination experience, preferred
