Qualifications / Job Requirements

• Active Registered Nurse (RN) license in California
• Minimum 1 year recent Case Management experience (RN Case Manager, clinical case management)
• Experience with Cerner EMR preferred
• Ability to work rotating weekends
• Strong communication, care coordination, and organizational skills

Key Responsibilities

• Coordinate and manage patient care across the continuum to ensure optimal outcomes (care coordination, utilization management)
• Collaborate with interdisciplinary teams — physicians, social workers, case managers, and ancillary staff — to develop and implement individualized care plans
• Advocate for patients and families, facilitating access to community resources and services
• Document clinical assessments, care plans, and discharge instructions accurately in Cerner EMR
• Participate in daily rounds, multidisciplinary discharge planning, and care transition activities
• Maintain compliance with regulatory standards, utilization review, and hospital policies
