Travel Physical Therapist Job Details

• Location: Fort Wayne, Indiana
• Setting: Outpatient physical therapy and rehabilitation
• Weekly Estimated Pay: $1,760–$1,890
• Start Date: October 19, 2026
• Contract Duration: 13-week travel contract
• Schedule: 5 shifts per week, 8 hours per shift
• Shift: 8-hour shifts; schedule may vary

Physical Therapist Job Requirements

• Degree in Physical Therapy from an accredited program
• At least 1 year of outpatient physical therapy clinical experience
• Current Physical Therapist license in Indiana or eligibility for Indiana licensure
• Current CPR certification
• Strong clinical reasoning, communication, organization, patient care, and documentation skills
• Ability to collaborate effectively with patients, families, healthcare professionals, and students
• New graduates are not eligible for this travel Physical Therapist opportunity

Outpatient Physical Therapist Responsibilities

• Complete comprehensive patient evaluations and identify functional limitations
• Develop, implement, and modify individualized physical therapy treatment plans based on patient needs, goals, and provider direction
• Establish measurable goals focused on functional progress and timely discharge
• Provide skilled physical therapy interventions that support a safe return to daily and community activities
• Document evaluations, treatment progress, goals, functional outcomes, medical necessity, and charges accurately and promptly
• Provide patient care in accordance with valid orders, applicable regulations, professional standards, and clinic policies
• Educate patients and families about treatment plans, home exercise programs, safety, and continued progress
• Participate in student education and supervision when assigned
• Maintain professional licensure and pursue ongoing clinical development and continuing education
• Support patient safety, employee safety, quality improvement, and collaborative healthcare teamwork
