Home Health Physical Therapist Job Details

• Work Environment: In-home patient care with travel between visits
• Additional Detail: Mileage between patient visits is reimbursed according to applicable policy

Physical Therapist Job Requirements

• Current, unrestricted Physical Therapist license in Massachusetts
• Master’s degree in Physical Therapy or a related field, as required by applicable standards
• At least 1 year of professional Physical Therapist experience, preferably in home health or home-based care
• Prior OASIS experience
• Strong interpersonal, communication, organizational, clinical, and documentation skills
• Ability to work independently, manage a daily home health visit schedule, and travel throughout the assigned service area
• Professional attire or scrubs appropriate for in-home patient visits

Home Health Physical Therapy Responsibilities

• Evaluate patients in their homes and develop individualized physical therapy treatment plans based on clinical findings, functional goals, and patient needs
• Provide skilled physical therapy interventions to improve strength, balance, coordination, mobility, range of motion, and functional independence
• Complete accurate OASIS assessments and timely clinical documentation in accordance with home health standards
• Monitor patient progress, modify treatment plans as needed, and communicate changes in condition to the appropriate care team members
• Educate patients and caregivers on therapeutic exercises, home safety, positioning, transfers, use of assistive devices, and injury-prevention strategies
• Coordinate care with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary healthcare team
• Manage travel between patient visits while maintaining dependable, punctual service
• Promote patient-centered care, clinical quality, and a positive experience in every home-based encounter
