Physical Therapist Job Details

• Weekly Estimated Pay: $2,260–$2,430

• Assignment: 13-week local contract
• Start Date: October 21, 2026
• Schedule: 8-hour day shifts, 5 shifts per week
• Hours: Approximately 40 hours per week
• Service Area: 19040, 19044, 18929, 18974, 18976, 19002, 18954, 18966, and 18901

Home Health Physical Therapist Requirements

• At least 2 years of Physical Therapy experience in a home health setting
• Demonstrated experience completing start-of-care evaluations and revisit visits
• Active Pennsylvania Physical Therapist license required
• Ability to provide reliable, patient-centered care in home-based environments
• Strong clinical judgment, communication, organization, and time-management skills

Physical Therapy Responsibilities

• Evaluate patients in their homes and develop individualized physical therapy plans of care
• Complete start-of-care assessments, follow-up revisit visits, and ongoing functional evaluations
• Provide therapeutic exercises, mobility training, balance activities, and functional interventions
• Educate patients and caregivers on treatment plans, safety strategies, home exercise programs, and progress toward goals
• Monitor patient response to treatment and modify interventions as clinically appropriate
• Document assessments, visits, outcomes, and care coordination activities accurately and promptly
• Collaborate with the broader care team to support continuity of care and positive patient outcomes
