Job Requirements

• Active Physical Therapist license in the state of Connecticut
• Doctoral degree required
• At least 1 year of Physical Therapy experience required
• Experience in home health, rehab, or outpatient therapy preferred
• Strong communication, organization, and clinical documentation skills
• Ability to work independently while contributing to a team-based care model
• Comfortable providing care in a home health setting
• Professional business casual attire or scrubs

Responsibilities

• Provide skilled physical therapy visits in patients' homes
• Evaluate patient needs and develop individualized treatment plans
• Complete timely, accurate clinical documentation and progress notes
• Collaborate with interdisciplinary team members to support patient goals
• Deliver compassionate, patient-centered care across assigned coverage areas
• Manage caseload efficiently while maintaining quality and productivity standards
• Support functional improvement, mobility, strength, and recovery through evidence-based therapy
