• Shift: Monday through Friday, approximately 8:30 AM to 4:30 PM
• Shift Duration: 8-hour shifts
• Hours per Week: 40
• Weekly Estimated Pay: $2,030 - $2,190
• Openings: 2
• Productivity Expectation: 30 points
• Weekend/On-Call Requirement: Not required
• Mileage Reimbursement: Yes, between patient visits

Job Requirements

• Active Physical Therapist license in the state of Connecticut
• Doctoral degree required
• At least 1 year of physical therapy experience preferred, ideally in home health, rehab, or a similar patient-care setting
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication, time management, and documentation skills
• Ability to travel between patient visits within the assigned coverage area
• Professional, patient-centered approach with a commitment to quality care

Responsibilities

• Provide home health physical therapy visits to patients across the assigned territory
• Perform assessments and develop individualized treatment plans based on patient needs
• Support patients in improving strength, balance, mobility, and functional independence
• Deliver timely, accurate documentation for each visit
• Collaborate with the interdisciplinary care team to coordinate patient care
• Educate patients and caregivers on exercise programs, safety, and recovery goals
• Manage a productive daily schedule while maintaining high standards of care
