Job Requirements

• Active Physical Therapist license in Massachusetts
• Minimum of 1 year of home care or home health experience required
• Experience working within a certified home health agency preferred
• Strong clinical judgment and the ability to work independently in the field
• Comfort traveling to patient cases as assigned throughout the Newton and greater Boston area
• Strong electronic documentation and communication skills
• Ability to collaborate effectively with patients, families, physicians, and interdisciplinary team members

Responsibilities

• Provide physical therapy services to patients in their place of residence
• Assess patients with physical impairments, functional limitations, or disabilities
• Develop and implement individualized treatment plans to support recovery and independence
• Monitor patient progress and adjust care plans as needed
• Coordinate care with physicians, families, and allied health professionals
• Document visits accurately and promptly in accordance with agency standards
• Communicate updates and patient needs to the interdisciplinary care team
• Travel to assigned cases across the designated coverage area
