RN Case Manager Job Details

• Healthcare Setting: On-site hospital facility

Registered Nurse Case Manager Qualifications

• Active California Registered Nurse (RN) license
• Minimum 5 years of case management nursing experience
• Proven ability to manage high patient volume (caseload ratio 1:25 or higher)
• Proficiency with Cerner EMR required
• Experience with InterQual preferred
• Previous experience in HCA hospitals or Southern California healthcare settings a plus
• Immediate availability to start
• Local candidates preferred for in-person interview; remote candidates will interview via TEAMS

RN Case Manager Responsibilities

• Coordinate and manage patient care plans for high-volume caseloads in a clinical setting
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• Utilize Cerner and InterQual for accurate documentation and patient progress review
• Advocate for patient needs and facilitate transitions of care
• Ensure compliance with hospital policies and regulatory standards
• Demonstrate strong organizational and communication skills in a fast-paced healthcare environment
