Registered Nurse Job Requirements

• At least 1 year of Registered Nurse experience required
• Previous homecare or home health nursing experience strongly preferred
• Experience with OASIS documentation required
• Experience with HCHB required
• Active Registered Nurse license in the state of New York
• Strong clinical assessment, documentation, and care coordination skills
• Ability to work independently and manage a patient caseload efficiently
• Reliable transportation for travel throughout the assigned territory

Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
• Complete patient assessments, develop care plans, and document visits accurately and thoroughly
• Utilize OASIS and HCHB systems to support compliant and timely clinical documentation
• Monitor patient progress and communicate changes in condition to the appropriate care team members
• Educate patients and families on treatment plans, medications, and disease management
• Coordinate care with interdisciplinary team members to support positive patient outcomes
• Participate in on-call and weekend coverage as needed
• Maintain productivity standards while delivering high-quality, compassionate home health care
