• Weekly Estimated Pay: $2290-$2660

• Location: Cordova, Tennessee

• Duration: 13 weeks
• Schedule: Monday-Friday, 8:00 AM-4:30 PM
• Shift Duration: 8 Hour Days
• Shifts Per Week: 5
• Hours Per Week: 40
• Coverage Area: Shelby, Fayette, and Tipton counties
• On-Call/Weekend Requirement: None
• Mileage Reimbursement: Reimbursed between visits

Job Requirements

• Hospice nursing experience required
• Experience with HCHB required
• Current active RN license required
• Strong case management and patient coordination skills
• Ability to work independently in a home-based care environment
• Flexible, easy-going, and professional demeanor

Responsibilities

• Provide skilled nursing care to patients in the homecare setting, including hospice-related support as needed
• Manage patient cases and coordinate care plans across the assigned coverage area
• Complete admissions and ongoing nursing assessments
• Prepare for and participate in interdisciplinary team meetings
• Document care accurately and efficiently in HCHB
• Support patients and families with compassionate education and communication
• Maintain productivity expectations of approximately 6 visits per day and 30 points per week
