Physical Therapist Job Requirements
- Bachelor’s degree in Physical Therapy
- Current Physical Therapy license
- BLS/CPR certification
- Valid driver’s license
- At least 1 year of relevant Physical Therapy experience preferred
- Home health experience strongly preferred
- Proficiency with HCHB preferred
Responsibilities
- Perform start-of-care (SOC), resumption-of-care (ROC), evaluations, and treatment visits
- Develop and implement patient-centered therapy plans in the home health setting
- Assess patient progress and adjust treatment plans as needed
- Document care accurately and in a timely manner in the EMR
- Travel throughout the assigned service area to provide consistent patient care
- Maintain strong communication with patients, families, and the care team
- Support quality outcomes through compassionate, skilled rehabilitation services
