Job Requirements
- Current Physical Therapist (PT) license and eligibility to practice in Washington state
- At least 12 months of home health experience within the past 3 years
- Minimum of 1 year of clinical experience required
- Strong assessment, treatment planning, and patient education skills
- Ability to work independently while managing a home health caseload
- Experience documenting in an electronic medical record (EMR) system preferred
Responsibilities
- Evaluate patients in the home setting and develop individualized treatment plans
- Provide skilled physical therapy interventions to support mobility, strength, balance, and functional independence
- Monitor patient progress and adjust plans of care as needed
- Educate patients and caregivers on exercises, safety, and recovery strategies
- Document assessments, treatments, and progress accurately and in a timely manner within the EMR
- Collaborate with the interdisciplinary care team to help ensure coordinated, high-quality patient care
