Physical Therapist Job Requirements

• Bachelor's degree required
• Current Physical Therapist license/certification
• At least 1 year of Physical Therapy experience preferred/required
• Prior home health experience strongly preferred
• Strong communication, organization, and time management skills
• Proficiency with HCHB preferred
• Ability to provide patient-centered care in a home health environment

Physical Therapy Responsibilities

• Assess and evaluate patient care needs related to mobility, functional status, endurance, stamina, pain, and other medical conditions
• Develop individualized treatment plans based on patient needs and clinical findings
• Perform therapeutic interventions that support improved mobility, independence, and overall well-being
• Monitor patient progress and adjust care plans as needed
• Collaborate effectively with the interdisciplinary care team to support positive patient outcomes
• Document visits and care plans accurately within the electronic medical record system
• Provide compassionate, high-quality care to patients in the home setting
