Physical Therapist Job Requirements

• Active Physical Therapist (PT) license
• Current CPR certification
• Master’s degree required
• At least 1 year of Physical Therapy experience preferred
• Home health experience strongly preferred
• HCHB experience is a plus
• Tech-savvy and comfortable using electronic medical documentation tools
• Professional, business casual appearance

Responsibilities

• Provide skilled home health physical therapy services to patients in their homes
• Evaluate patient mobility, strength, safety, and functional progress
• Develop and implement individualized treatment plans
• Educate patients and families on exercise programs, safety, and recovery goals
• Document visits accurately and in a timely manner
• Coordinate care with the interdisciplinary healthcare team to support patient outcomes
• Travel between patient visits within the assigned coverage area
• Maintain a patient-centered approach focused on quality care, independence, and recovery
