• Schedule: Monday through Friday, 8-hour day shifts, approximately 32 hours per week
• Estimated Weekly Pay: $2,030 - $2,190
• On-Call/Weekend Requirement: None
• Coverage Areas: Christian, Trigg, Caldwell, and Lyon counties
• Patient Care Focus: Home-based rehabilitation, mobility support, and functional recovery

Job Requirements

• Active Physical Therapist license or eligibility to practice in Kentucky
• Doctoral degree in Physical Therapy required
• Home health experience preferred
• Strong clinical skills in evaluating mobility, function, and safety in the home setting
• Experience with HomeCare HomeBase preferred
• Excellent communication, time management, and patient education skills
• Professional, dependable, and comfortable working independently in the field

Responsibilities

• Evaluate patients in the home setting and develop individualized plans of care
• Support patients in improving physical function, mobility, and safety at home
• Provide skilled therapy interventions aligned with each patient's goals and progress
• Educate patients and caregivers on exercises, safety strategies, and functional mobility techniques
• Document care accurately and timely according to home health standards
• Coordinate with interdisciplinary team members to support optimal patient outcomes
• Manage a productive schedule while maintaining high-quality, patient-centered care
