• Weekly Hours: 40 hours per week
• Shift Duration: 8-hour shifts
• Schedule: Monday–Friday, 8:00 AM–4:30 PM
• On-Call Requirement: 1 weekend per month
• Weekly Estimated Pay: $2,030–$2,190
• Dress Code: Navy scrubs
• Coverage Area: Dallas service area

Job Requirements

• Active Physical Therapist license in the state of Texas
• Doctoral degree required
• At least 1 year of home health experience
• Experience using HCHB preferred and strongly valued
• Comfortable working independently in a home health therapy setting
• Experience with post-op orthopedic patients is a plus
• Strong communication, organization, and time-management skills

Responsibilities

• Perform OASIS evaluations and subsequent patient visits
• Develop and carry out individualized treatment plans based on patient needs
• Provide skilled physical therapy services in patients’ homes throughout the Dallas service area
• Document care accurately and efficiently in the required system
• Collaborate with the care team to support positive patient outcomes
• Maintain productivity expectations of approximately 30 points per week
• Participate in on-call coverage as scheduled, including 1 weekend per month
• Deliver compassionate, patient-centered care with a focus on functional progress and recovery
