Job Requirements
- Active Physical Therapist (PT) license or eligibility to practice in Wisconsin

- Familiarity with HCHB preferred
- Strong communication, organization, and time-management skills
Responsibilities

- Provide skilled physical therapy interventions to support mobility, safety, and functional progress
- Track patient outcomes and adjust care plans as needed
- Educate patients and caregivers on therapy goals, exercises, and home safety strategies
- Document visits accurately and timely in the EMR system
- Collaborate with the care team to help ensure coordinated, high-quality patient care
