- Mileage: Reimbursed between patient visits

Job Requirements
- Bachelor's degree in Physical Therapy or a related field
- Current Physical Therapist license
- CPR certification required
- At least 1 year of Physical Therapy experience preferred, ideally in home health, outpatient, or rehab
- Home health documentation experience preferred; HCHB experience is a plus
- Strong communication, organization, and time-management skills
- Ability to work independently while delivering high-quality patient care
Responsibilities
- Provide skilled Physical Therapy services to patients in a home health setting
- Develop and implement individualized treatment plans based on patient goals and needs
- Assess patient progress and adjust care plans as appropriate
- Educate patients and caregivers on therapy goals, exercises, and safe mobility techniques
- Document evaluations, treatments, and patient outcomes accurately and timely
- Coordinate care with physicians, nurses, and the broader clinical team to support positive patient outcomes
- Travel between patient visits within the assigned coverage area in Easton and surrounding Maryland communities
