Job Requirements

- Comfortable documenting in Cerner
- Strong communication, organization, and time-management skills
- Reliable transportation for home visits throughout the Columbia, TN area
Responsibilities
- Assist Physical Therapists in delivering skilled home health physical therapy services
- Carry out treatment plans designed to improve patient mobility, strength, balance, and function
- Provide patient education to support safe recovery and progress at home
- Document visits accurately and efficiently in the electronic charting system
- Collaborate with the care team to support high-quality patient outcomes
- Adapt to a patient census of approximately 6-10 visits
- Maintain a professional, compassionate approach in every home setting
