• Start Date: August 17, 2026
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Shift Length: 8 hours
• Hours per Week: 40
• Weekly Estimated Pay: $2,120 - $2,230
• Dress Code: Navy blue preferred

Registered Nurse Job Requirements

• Active Registered Nurse (RN) license required
• Hospice RN experience strongly preferred
• Home health or homecare experience preferred
• Associate degree in nursing or higher required
• Experience with HCHB is an advantage
• Strong patient-centered approach with a positive, flexible attitude

Responsibilities

• Case manage a group of hospice patients in the homecare setting
• Coordinate and deliver compassionate, high-quality nursing care
• Support patients and families with clinical guidance and ongoing communication
• Complete nursing assessments, care planning, and patient follow-up
• Collaborate with the interdisciplinary care team to help ensure continuity of care
• Travel between patient visits within the coverage area as needed
• Maintain accurate documentation and uphold professional standards of care
