Physical Therapist Job Requirements

• Active Physical Therapist license
• Master's degree required
• At least 1 year of physical therapy experience
• Home health experience preferred
• HCHB experience preferred
• Strong communication, documentation, and time-management skills
• Ability to work independently while collaborating with the interdisciplinary care team

Responsibilities

• Evaluate patients' functional status and physical therapy needs in the home health setting
• Assist in developing individualized plans of care with the physician and care team
• Provide physical therapy services within scope of practice and state guidelines
• Monitor patient response to treatment and communicate updates to the care team
• Treat patients to relieve pain, restore function, and support maximum independence
• Educate patients, families, and caregivers on treatment plans and progress toward goals
• Complete accurate patient evaluations, visit notes, and progress documentation
• Report evaluation outcomes, care goals, and anticipated frequency of visits
