Job Requirements
- Active Registered Nurse (RN) license in good standing
- Minimum of 2 years of home health nursing experience
- Experience completing OASIS assessments and care planning for in-home patients
- Strong clinical judgment, organization, and communication skills
- Ability to work independently while coordinating effectively with the care team
- Experience with Epic is a plus
Responsibilities
- Provide skilled nursing care to patients in their homes
- Complete comprehensive OASIS assessments and develop individualized care plans
- Monitor patient progress and communicate updates to the interdisciplinary care team
- Educate patients and families on care needs, safety, medication guidance, and follow-up instructions
- Participate in the department call rotation as scheduled, including nights, weekends, and holidays
- Maintain accurate, timely, and compliant clinical documentation
