• Shift Duration: 8-hour shifts
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Shifts per Week: 5
• On Call / Weekend Requirement: None
• Weekly Estimated Pay: $2,030 - $2,190
• Mileage Reimbursement: Provided between patient visits
• Coverage Area: Home visits throughout nearby Northern Virginia communities

Job Requirements

• Active Virginia Physical Therapist license required
• Minimum of 1 year of Physical Therapy experience required
• Previous Home Health Physical Therapy experience required
• Experience with HomeCare HomeBase (HCHB) documentation required
• Experience with PointCare documentation required
• Strong communication, organization, and time-management skills

Responsibilities

• Provide skilled physical therapy services to patients in the home health setting
• Evaluate patient needs and develop individualized treatment plans
• Document care accurately and efficiently using HCHB and PointCare
• Track patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy goals, exercises, and safety
• Travel between scheduled visits throughout the assigned service area
• Maintain productivity standards while delivering high-quality patient care
