Travel Physical Therapist Job – Home Health in Dallas, TX 75240

- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- On-Call Requirement: 1 weekend per month
- Productivity Expectation: 30 points per week
- Mileage Reimbursement: Between patient visits
- Estimated Weekly Pay: $2,030 - $2,190
Job Requirements
- Active Physical Therapist license in the state of Texas
- Doctoral degree required
- At least 1 year of home health physical therapy experience
- Experience with HCHB preferred
- OASIS documentation experience strongly preferred
- Experience with post-op orthopedic patients is a plus
- Strong communication, organization, and time-management skills
- Ability to work independently and manage a home health caseload
Responsibilities
- Provide skilled physical therapy care in the home health setting
- Complete OASIS assessments and subsequent patient visits
- Develop and implement individualized treatment plans based on patient needs
- Evaluate patient progress and adjust care plans as needed
- Document care accurately and in a timely manner using HCHB
- Collaborate with the care team to support quality patient outcomes
- Educate patients and caregivers on therapy goals, exercises, and safety
- Support weekend on-call coverage as scheduled
