Occupational Therapist Job Requirements

• Minimum 1 year of Occupational Therapist experience required
• Graduate of an accredited Occupational Therapy program
• Active, unrestricted Occupational Therapy license in the state of practice
• Current CPR/BLS certification
• Home health Occupational Therapy experience preferred
• Experience with OASIS documentation, home health regulations, and Conditions of Participation preferred
• Familiarity with functional assessments, fall prevention, ADL/IADL training, home safety evaluations, and adaptive equipment recommendations preferred
• HomeCare HomeBase (HCHB) EMR experience preferred
• Bachelor’s degree required

Occupational Therapy Responsibilities

• Perform discipline-specific evaluations to determine the need for occupational therapy services
• Develop and implement individualized occupational therapy plans of care
• Train patients in activities of daily living (ADLs) to support independence at home
• Provide caregiver education to improve patient support, safety, and continuity of care
• Complete accurate and timely documentation in accordance with home health standards
• Conduct reassessments for home health patients as needed
• Complete admission and comprehensive assessments when therapy services are included in the referral and skilled nursing is not ordered
• Coordinate care with other disciplines to support positive patient outcomes
